Yeah she's dead. The title of the doujin translates literally as "A book where terrible things are done to Marisa and then she's buried." She's being used as a sacrifice by local villagers to bless their village and they decide to "use" her beforehand. At the end they realize she's dead but say it's fine since she's probably pregnant, even if she's dead the new baby inside her can stand in as the sacrifice.
