Not knowing anything about dragon quest or knowing Japanese it was still fun to try to guess what was going on on this one. It seems guy saves "girl". Guy wants "girl" until a big bulge raises "her" skirt. He is about to graciously walk until magical being makes "her" D irresistible. After guy sucks the D he awkwardly retreats. "Girl" wants more and does some research goes and ties him up in his sleep, wakes him up and lubes his hole (I do appreciate this point as in a lot of stories apparently assholes are able to take huge dicks with no lube without a hospital visit oh and somehow get off on it too smh) and has "her" way with him. Overall an 8 out of 10. I use "her" as the artwork doesn't make clear if "she" has a vagina or not. Without the V it is a guy with boobs, with a V it is a girl with a D.
